Expired Medical Cards Now Park More Drivers Than Hours of Service

A piece of paper is now the top reason drivers get parked

The Commercial Vehicle Safety Alliance released the driver side of its 2026 International Roadcheck results on August 25, and the ranking changed at the top. For years the headline driver violation was hours of service. Not this year. The most common reason a driver was placed out of service during the three day inspection push in May was not carrying a valid medical examiner's certificate. Land Line Media and Heavy Duty Trucking both reported the breakdown on August 26.

Inspectors ran 54,575 inspections across North America on May 12 through 14 and wrote 3,969 driver out of service violations. Missing medical cards accounted for 1,072 of them, right at 27 percent of the total. In the 2025 event the same category produced 493 violations and 14.9 percent. That is more than double the count in a single year, and it moved medical cards past hours of service into first place.

The rest of the driver top five shifted too

Hours of service fell to second with 929 violations, 23.4 percent of driver out of service violations, down from 32.4 percent a year earlier. No CDL came third at 620 violations and 15.6 percent, down from 24.4 percent. English language proficiency entered the top five for the first time at 361 drivers and 9.1 percent, its first year as an out of service condition during Roadcheck. False records of duty status finished fifth at 266 violations and 6.7 percent.

One detail is worth sitting with. The announced driver focus for the 2026 event was electronic logging device tampering, falsification and manipulation. Enforcement went looking for sophisticated log fraud. What it found most often was expired paperwork.

Why an expired card follows you to renewal

An out of service order is not just a bad afternoon. It goes into the federal record behind your DOT number, and a missing or expired medical certificate lands in the Driver Fitness category of the Compliance Safety Accountability system. Underwriters read those categories. When a small fleet comes up for renewal, the file on the desk is your loss runs, your motor vehicle records, your radius, your unit count, and your CSA percentiles. A Driver Fitness score that moved the wrong way is a rating factor long before it is ever a claim.

That matters more for a two truck operation than for a two hundred truck one. With a handful of power units there are very few inspections to average against, so one out of service order carries real weight in the percentile math. The cheapest violation in trucking to avoid

Almost everything else on that top five list costs real money or real downtime to fix. A DOT physical does not. The exam is routine, the certificate can run up to two years when nothing complicates it, and the entire failure mode is forgetting an expiration date.

Three habits close the gap. Put every driver's certificate expiration on a calendar with a reminder ninety days out rather than thirty, because appointments with certified examiners fill up and your state can downgrade a CDL once it no longer has a current certificate on file. Keep one copy in the truck and one in the office, since the roadside version of this conversation goes a lot faster when the driver can hand it over. And book the recertification before the old card lapses instead of after, because the gap between the expiration and the new exam is exactly the window where a routine inspection turns into an out of service order.
